To start the pre-operative surgical assessment process, Naperville Surgical Centre requests that you fill out your medical history online with One Medical Passport. We recommend that you enter your medical history as soon as your surgery has been scheduled. Once you do this, our Pre-Operative Assessment Nurse will be able to access the information you entered online. This information will assist the nurse in organizing and documenting your complete medical history to prepare for your surgery.
Please be certain to have the following information available prior to starting your One Medical Passport:
- The names, addresses, and phone numbers of your primary care physician and any specialists.
- A list of all medications you are currently taking, including their dosage and frequency.
- A list of surgical procedures you have ever had.
- The name of the surgeon who will be performing your upcoming procedure.
At the end of the data entry process you will be prompted to select your preoperative instructions based on the type of anesthesia your surgeon said you will have. Please select and print the instructions, and have them available to refer to when our nurse calls you before your surgery. You may want to make some notes during this phone call.

About One Medical Passport:
Completing a One Medical Passport medical history online is easy. For most patients, filling out the entire questionnaire takes less than 30 minutes. Please fill out the questionnaire accurately, and be assured that your information is confidential and will be thoroughly reviewed by your medical team prior to your surgery or procedure. Don’t worry about being interrupted during your process as you can come back and complete the unfinished portion of your One Medical Passport at any convenient time.
One Medical Passport is a website that allows you to enter your information at any time, from anywhere. You can also print a copy of your after you create it online and keep it with your other healthcare documents. Future access is available at any time, from any location with an internet connection, should you want to update or re-print your One Medical Passport.
Information Security: One Medical Passport uses the highest grade of internet security available. Full data encryption with SSL [secure socket layer] technology. Your information is stored on secure servers, at a secure webhosting site, and will be released only to your medical team at Naperville Surgical Centre. Your information is never released to anyone other than your medical team.
Surgical Consent Form
The goal of informed consent is to establish a mutual understanding between the patient and the physician/surgeon about the surgical procedure, care, and treatment you will receive. This process allows you to fully participate in decisions about your surgery and care.
Before you arrive at the surgery center, your surgeon will have discussed with you the condition requiring treatment and the nature, purpose, risks, and benefits of the operation/procedure, possible alternative methods of treatment, possible results of no treatment and possibility of complications. Your surgeon will have given you the opportunity to ask questions and have them answered to your satisfaction.
